PittPharmacy_FrontPavilion_0247PittPharmacy students Hetvie Kaashyap and Kristen Conn( Class of 2016), presented posters at the Aging Institute’s 9th Annual Research Day held on March 30, 2015 at the University Club at the University of Pittsburgh. This year’s poster session included  presentations by clinicians and researchers from throughout the UPMC and University of Pittsburgh systems

Kaashyap and Conn presented their work on improving care transitions for older adults. Kaashyap’s project was entitled “Development of a Medication Tracking Tool for Transitions of Care” and Conn’s project was entitled “Quality Improvement of Transitions of Care Follow-up Process”.

The students’ special topics projects were mentored by Christine Ruby-Scelsi, PharmD BCPS, assistant professor in the pharmacy and therapeutics department in the School of Pharmacy. Ruby-Scelsi is also a clinical pharmacy specialist in Geriatrics at the Senior Care Institute at UPMC Shadyside.
