Instructions for State Board Applications

The Pennsylvania State Board and Out of State Board application process is below. Please take time to READ everything. There are several changes with the PA State Board and new requirements to be eligible to receive your PA State Board Licensure. The process in its entirety, including step-by-step instructions and a checklist, will be communicated to eligible students in their graduating term.

Beginning January 1, 2015, all health-related licensees are considered “mandatory reporters” under section 6311 of the Child Protective Services Law (CPSL) (23 P.S. § 6311), as amended. Therefore, all applicants must complete a 3 hour course requirement on child abuse reporting. The link to the Department of Human Services’ (DHS) web site is: http://keepkidssafe.pa.gov/training/index.htm. When you use the link, hover over Resources and click on “Training for Mandated Reporters”. You can view the list of DHS-approved programs under Training Info. The very first one: www.reportabusepa.pitt.edu is free of charge and this program provider electronically submits information to the Board. Pennsylvania State Board Process: As of December 13, 2017, the following Pennsylvania State Board of Pharmacy (Board) applications are online only applications:  Pharmacist License by Exam, Score Transfer and Reciprocity, and the Authorization to Administer Injectables.  The paper applications were removed from the Board’s web site and will no longer be accepted.  Students must individually apply for a license at www.pals.pa.gov.

Step 1: Complete the “Recognizing and Reporting Child Abuse” online training program
Step 2: Obtain the National Practitioner Databank Self-Query Report*
Step 3: Apply for the Pharmacist License*
Step 4 (optional): Apply for the Authorization to Administer Injectables License*
Step 5: Obtain necessary Criminal History Check(s)*
Step 6: Apply to take the NAPLEX and MPJE*
Step 7: Register for a date to take the NAPLEX and MPJE*
*fee, subject to change

The Certificate of Pharmacy Education and Pharmacy Intern Experience (Controlled Internship Program) Form MUST be downloaded and submitted to the School of Pharmacy before the school’s published deadline. Forms generated by the application process contain a bar code that is unique and specific to the applicant. They cannot be duplicated by use for other applicants. Any late responses will delay in the processing of your PA State Board Application. The School of Pharmacy will use the information from the Certificate of Pharmacy Education Form to complete the Verification of Opioid Education form on your behalf. We will forward the signed and certified forms to the State Board 30 days prior to graduation. Please scan and email your form to CRS149@PITT.EDU or mail them to the address below:

If you are NOT seeking the authorization to administer injectable medications, biologicals and immunizations, you must email Cheryl at crs149@pitt.edu informing that you are NOT seeking this license.

 

Out of State Licensure Application Process:

It will be your responsibility to forward your out of state applications to the school for completion. The applications must be emailed to Cheryl at crs149@pitt.edu or mailed to the address below before the school’s published deadline. Any late responses will delay in the processing of your State Board Application. When the applications are completed with appropriate hours and signatures, they will be forwarded to the appropriate state after graduation.

Also, once you receive your ATT Number from the State Board must register to take the NAPLEX and MPJE at the National Association of Boards of Pharmacy’s (NABP’s) website – www.nabp.net.
